Confined Space Entry on Location: Tanks, Pits, and the Permit
A mud tank needs a shovel and a light. A frac tank needs a washdown. The cellar under the rig floor needs a check after a rain. Every one of those jobs looks like a quick errand, and every one of them is a confined space entry. The opening is small, the ventilation is poor, and the atmosphere in an oilfield tank can be lethal long before anyone smells a problem. The crews who die in tanks are almost always the ones who went in without a permit, and the second victim is almost always the person who tried to pull them out.
The atmosphere is the hazard you cannot see
Hydrogen sulfide is heavier than air and pools in tanks, pits, and cellars. Hydrocarbon vapor settles the same way, and it does not need a spark to be dangerous; it needs a flame or a static discharge. And oxygen can be displaced by inert gas, by rust, by biological activity in a water tank, leaving a space that reads normal and kills in three breaths. The test sequence matters as much as the test. Oxygen first, because the meter itself can be wrong in a low-oxygen atmosphere. Flammability second. Toxics third, with the sensor chosen for what is actually in that tank, not a generic reading. And the test has to happen with the space at rest, before ventilation changes the answer, because testing after the blower has run tells you about the blower, not the tank.
The permit is a rescue plan, not paperwork
The entry permit has to name the space, the work, the entrants, the attendant, the supervisor who authorizes, the atmospheric readings and when they were taken, and the isolation steps: lines blanked or disconnected, agitators locked out, contents drained. The section that separates a real permit from a stamped form is rescue. Who pulls the entrant out, with what equipment, and how fast? On a rig the honest answer is often "the crew with a rope," which is how rescue attempts become multiple fatalities. The emergency response plan on this site has to name the rescue method before the permit is signed, and a tripod, harness, and winch cost far less than the alternative.
API's recommended practice for confined space entry in the petroleum industry, RP 2217A, goes beyond the OSHA minimums for the specific case of tanks and vessels that have held hydrocarbons. The principle it repeats is the one crews forget: a tank that held oil is never "empty" for entry purposes, because the film on the walls, the scale in the bottom, and the vapor in the void all remain.
Who counts as an attendant
The attendant is a full job for the duration of the entry, not a task you give to the person who is also running the forklift. The attendant stays at the opening, keeps visual or voice contact, monitors the space, and initiates rescue when the entrant does not answer. If the attendant is also doing anything else, the entry is not permitted, full stop. That single rule, enforced, prevents most of the bad outcomes, because the person who would have gone in after a fallen entrant is instead the person who calls for help with the retrieval line in hand.
The chemical inventory and HazCom records on this site tie into the same moment: the entrant needs to know what was in the tank, and the SDS tells the attendant what the rescue risk actually is. A tank that held a biocide is a different rescue than a tank that held water, and the difference has to be on the permit.
