Electrical panels on a rig are not background scenery. They feed drawworks, pumps, top drives, shale shakers, living quarters and control systems. A technician who closes a breaker while someone else is troubleshooting a motor has just crossed from maintenance into injury. Lockout and tagout exists to prevent that handoff from happening by accident, and it works only when every step is followed, not remembered. The first step is energy identification. A qualified person must list every energy source that could reach the equipment: main power, stored capacitor charge, auxiliary circuits, hydraulic accumulators, pneumatic pressure and spring tension. Once those sources are named, each one gets a lock, a tag and a physical separation. On a rig, multiple crafts often work on the same system, so group lockout boxes and multiple locks on the same hasp keep everyone honest. Removing a lock before the work is complete is the single behavior that causes most LOTO injuries. Verification is the step most crews skip. After locks and tags are placed, a qualified person must attempt to start the equipment with normal controls and confirm that nothing moves. That test is not optional. A control room start button that does nothing does not prove the motor is isolated if an accumulator can still drive the actuator. Procedure documentation and training keep LOTO from becoming guesswork. Each panel or system should have a written procedure that names the isolation points, lock locations, verification steps and the person responsible for lock removal. New hires and visiting contractors need that procedure reviewed before they touch anything. A lock left on overnight without a tag that explains why is a problem for the next shift. A tag that lists a task completed three days ago is another problem. When lockout and tagout is treated as paperwork instead of life safety, the paperwork becomes useless. Enforcement matters. Supervisors who see a lock hanging without paperwork should stop the job, not hope the worker remembers the task. Contractor management is especially important because outside crews bring different habits and different assumptions about what isolated means. A contractor who follows one company's LOTO program will fail another's if the steps are not reviewed on site. The easiest way to keep that from happening is to attach the written procedure to the permit, review it at the job setup, and keep a copy in the hands-on language of every craft involved. A program that survives audits and inspections is a program that protects the same crew every day. The cost of a single LOTO failure is usually higher than the entire training program.
